What is Cymbalta?

(Duloxetine, Ariclaim, Yentreve, Xeristar)

Cymbalta (Duloxetine) is a Serotonin Norepinephrine Reuptake Inhibitor Antidepressant. The half-life is about 12 hours (range 8-17 hours) with an average 2-hour lag until absorption begins. Maxium plasma concentrations occur about 6 hours post dose. Food can delay the time to reach peak concentration.  Cymbalta is highly bound to proteins in the blood, binding primarily to albumin, a protein made by the liver. This may explain why the FDA experts conclude that Cymbalta can cause hepatoxicity in the form of transaminase elevations, which are an indicator of liver damage.  The liver toxicity warnings are included in the prescribing information.  There have been numerous reports of adverse events occurring upon discontinuation of Cymbalta. This is particularly true when abrupt or rapid reduction of the dosage occurs. The manufacture recommends a gradual dose reduction rather than abrupt cessation whenever possible.
